- You are responsible for full payment of all fees incurred with our physicians. While we file your insurance as a courtesy, this does not relieve you of payment responsibility.
- Regarding disputes between divorced parents of a child we treat: We do not intervene. The parent bringing the child is responsible for full payment; we do not bill the other parent.
- For children 18 years or older on parental insurance: A full-time student status form is required. Until received, you're responsible for payment in full.
- We do not file Medicare supplemental insurance policies. Annual deductibles and coinsurance are your responsibility. Refunds for secondary policy payments are processed around the 15th of each month.
- You must obtain any required referral from your Primary Care Physician (PCP) before your visit. Without a valid referral, full payment for services is expected.
- While most patients are familiar with their insurance benefits, as specialists, changes may occur in coverage.

Ears: We offer medical and surgical treatment for ear infections, hearing loss, ear deformities, and tumors.
Nose: Our expertise covers sinus issues, nasal allergies, nose deformities, snoring/sleep apnea, and nasal tumors.
Throat: We address vocal/speech/swallowing problems, airway/breathing issues, and neck/throat tumors.

For all inquiries or concerns, including medication refills and appointments, please contact our office at 281-265-1160. Our receptionist or nurse will assist you and answer most questions. If the doctor's opinion is necessary, expect a return call. Please note, that medication cannot be prescribed without a recent evaluation, and most requests will require an office visit. Ensure your pharmacy has our contact information for refill requests during regular office hours.

- To cancel appointments, please call 281-265-1160. If you don't reach the receptionist, you can leave a detailed message on the voicemail. Please note, cancellations cannot be made via email.
**Late Cancellations and No Show Policy:**
- Late cancellations will be treated as a "no show" unless the appointment was made within less than 24 hours.
- A "no show" refers to missing an appointment without canceling it by 10 a.m. one (1) working day in advance. This inconveniences others needing timely medical care.
- Failure to attend a scheduled appointment will be noted as a "no show" in the patient's chart, and a fee of $30.00 will be billed.
